Advanced illness management was created to help patients with complex illnesses remain safely at home and avoid unnecessary hospitalization. Our clinical teams work hand-in-hand with your physician to provide ongoing care and support in your place of residence, wherever that may be. From comprehensive assessments and remote monitoring by phone, to in-person visits and after-hours visits when needed, our goal is to improve your quality of life wherever you call home.
Every patient has the right to make decisions related to medical treatment, including the right to accept or refuse medical care. Our team will work with you to document your wishes, establish your personal care goals, and help you achieve them.
Your comfort is of utmost importance, and comfort starts with understanding your goals. Your nurse or nurse practitioner will discuss pain and other symptoms (such as shortness of breath, fatigue, constipation, loss of appetite, nausea and vomiting, and difficulty sleeping) with you and your caregiver and develop a symptom management plan with you and your physician.
Our team is an in-home extension of your physician, who will continue to coordinate your care. We will also collaborate closely with other healthcare providers to fulfill the plan of care that meets your care goals. Together, we will help ensure that you are in the right care setting to meet the needs of your condition.

Advanced illness management is a specialized form of high-touch palliative care offered through contracted relationships with physicians.
Advanced illness management and hospice care are both designed to help people diagnosed with a serious or life-limiting illness, but they have two distinct differences. Advanced illness management may be provided at any stage of a serious illness, while hospice services require a terminal prognosis. Advanced illness management can also be provided at the same time as curative treatments and therapies, while hospice care requires patients to forgo curative treatment.
Your trusted physician remains actively engaged in your care, collaborating with our clinical team to ensure you receive the appropriate treatments for your symptoms. Your nurse practitioner will closely monitor your condition and symptoms, providing in-person care, evaluating your condition, reconciling medications, and assisting with advanced care planning. Care managers monitor your condition remotely. They will reach out regularly to assess your condition, identify any new or worsening issues, and work with your nurse practitioner to adjust your care when appropriate.
Because not all pain is physical, our social workers help guide you and your family through the emotional challenges that may accompany a serious illness. They can also connect you with additional support and resources within your local community. Finally, after-hours nurses are available on nights, weekends, and holidays to ensure that no matter when you need support for worsening symptoms or a health crisis, you get the care you need.

Our care is provided to you through a relationship with your healthcare provider or health plan at no additional charge to you, in most cases.
Our care is focused on helping you improve your quality of life. In some cases, this can improve your response to existing medical treatments. It also gives you a team of experts to help you navigate the emotional and psychological challenges brought on by a serious illness or life-limiting condition. Advanced illness management can be provided in a private home, in assisted living, or in a skilled nursing facility setting.
Advanced illness management works with your current healthcare providers to ensure that each aspect of your treatment plan and care is communicated and coordinated effectively. It’s also unique in that it offers more contact with clinicians and a uniquely collaborative approach to assisting patients with their serious illness journey by helping relieve symptoms that may include physical or emotional pain, fatigue, loss of appetite, difficulty sleeping, or depression related to serious illness.
Once your physician provides a referral for our care, an Illumia Health clinician will visit you wherever you call home. Working with you and your doctor, we'll design a plan of care that addresses your unique health needs — and we'll continue to collaborate with your physician and modify your care plan if your needs change.
